PURPOSE AND KEY FOCUS


The Licensed Practical Nurse (LPN) will work with the Community Care Program, and with Home Care
Nurse to provide nursing care as part of an interdisciplinary team within the context of a
client-centered, community-based setting. Within the community of Okanagan Indian Band, the LPN
will provide comprehensive and collaborative nursing functions and services focusing on and
community and/or public health through the promotion and maintenance of individual, family, group
and community health – with a focus on Senior’s care.

JOB SUMMARY


The LPN will work with a team that values the principles of primary health care and focuses on
promoting health, preventing disease and injury, as well as focus on emphasizing the independence
of the client. The LPN will assist and encourage OKIB community members to identify their own
health needs to develop a care plan and taking appropriate action to ensure the health needs are
being met.

REPORTING STRUCTURE


Reports to the Home Care Nurse RN (Clinical Oversight) and Home Care Supervisor (Administrative
oversight)

JOB DUTIES AND RESPONSIBILITIES


▪ Provide support to the Home and Community Care Registered Nurse (RN)
▪ Walk-in Care services, conduct home/office visits as required
▪ Facilitate and co-facilitate Case Conferences
▪ Immediately report to a higher level of nursing care and change client care plans in
coordination with the Home and Community Care RN
▪ Advocate for leading practices and implement changes that improve services and quality
assurance
▪ Work with Home Care Nurse in ongoing evaluation of client care plans
▪ Collaborate with internal and external health professionals to deliver health education and
services to clients and community through home visits and clinics
▪ Ensure confidentiality is maintained for all client files, records, and
communications
▪ Inform team members of relevant, up-to-date education, policies, and procedures.
▪ Provide culturally sensitive and comprehensive palliative care to clients at end of
life.
▪ Assess community health needs using local knowledge and information, morbidity, mortality, and
general health status statistical information.
▪ Monitors the health throughout all the ages, with importance placed on at risk groups, through
assessment, screening clinics and early intervention for identified health problems
▪ Promotes a safe and healthy environment in the community, homes and workplace.

  • All persons employed by the Okanagan Indian Band may be required to assist the Band in providing
    emergency services. Duties assigned during an emergency may differ from regular duties.

EDUCATION AND EXPERIENCE


▪ Licensed Practical Nursing Diploma
▪ Current, active registration with BCCNM to practice nursing in British Columbia
▪ Training in wound care management
▪ Training communicable disease management
▪ Current First Aid/CPR certificate level C
▪ Valid BC Driver’s licence, minimum Class 5 and reliable vehicle
▪ Minimum two years’ experience in nursing
▪ Managing clients with health conditions throughout the ages
▪ Experience working in a clinical setting and community setting
▪ Experience working with First Nations communities and/or populations
▪ Experience with Meditech System

CLASSIFICATION/PAY SCALE
$34.00 – $40.00/Hourly dependent on education and experience
